Since upgrading to SP1 RTM, I have seen the following alerts show up and they do not seem to go away or close: OleDb Module encountered a failure 0x80004005 during execution and will post it as output data item. Unspecified error Workflow name: Microsoft.SystemCenter.SqlBrokerAvailabilityMonitor Instance name: rtprms01.americas.ppdi.local Instance ID: {5DE412EC-E51D-8DBA-0E8A-3AC5EE2BE221} Management group: US-MGMTGRP Any ideas? Hi there This is a permission issue with the possibily the management action account or SDK to the "master" database. Try granting local admin rights to these accounts and see how it goes. I'm seeing this error as well. The management server action account, SDK account, & data writer accounts are all local admins.
