Access violation in D3D9.DLL   

would like to know under what situation would such occur, as i run the 
Dependency Walker:

Second chance exception 0xC0000005 (Access Violation) occurred in 
"c:\windows\system32\D3D9.DLL" at address 0x00DB8732 by thread 1.

i'm using DirectX 9.0c (4.09.0000.0904)
