Workflow, sometimes it runs, sometimes not, sometimes hours later - where do 
I start?

There is service on CRM server MSCRMAsyncService , check it as it
process the queue items.
Check the system jobs
Open the workfllow and see what was problem or what made them not to
fire .
re-check the creteria you have made to fire the workflow

Gavin

We have had no end of problems with Workflow since we started
development in CRM 4. There is a hotfix due this month (I think it's
out for beta testing at the moment with us) and hopefully this will
sort out a whole raft of problems with the Workflow processes.

topic, how reliable have you found the CRM Email Router to be?

Microsoft issued the hotfix , which is not  publicly available. You
need to contact the support to get it.
There is another post from microsoft crm online partner about this
issue and hotfix on this forum.

Hi Gavin,

I think you have configured async workflows so try configuring sync workflow 
in order fire it after the specified action.
You can also check the status of workflow in entities workflow tab.

Hi, i am having problems too with my workflows. Does anybody where can we get 
the hotfix from? I did some research on it, i only found one for the 
workflows coming from the deployment manager. That's not my case, i need one 
for the workflows that are imported of that are built on the system. Any idea?
