HIPAA 3.3 Solution on R2   
I have a solution that was developed on Biztalk 2006 for use with the
HIPAA 3.3 Accelerator.  Will this solution work as-is if deployed to a
newly installed Biztalk 2006 R2 server?  The R2 server was a fresh
install and was not an upgrade of plain 2006.  Furthermore, because
it's a fresh install, it currently has no solutions deployed to it.

One more thing... just to elaborate...  The new Biztalk 2006 R2 server
does not have HIPAA 3.3 installed.  In fact, the documentation said
that functionality had been incorporated into the EDI/AS2 base
functionality in R2.  Hence my question...

They are two completely different beasts.
The schemas are different, and the unerlying process is completely 
different.

"The Microsoft BizTalk Accelerator for HIPAA 3.3 is no longer supported with 
the R2 release of BizTalk Server. However, support for the HIPAA accelerator 
is provided for customers who have the accelerator installed on BizTalk 
Server 2006 and who are upgrading to BizTalk Server 2006 R2. The HIPAA 
accelerator is supported only for upgrade scenarios; it is not supported as 
a new installation under BizTalk Server 2006 R2 because HIPAA functionality 
is provided under EDI/AS2."

Thanks!  I suspected that was the case but was not certain.

Lastly, when I try to download the Windows SDK, it takes me to the
Vista SDK.  I'm thinking I need the Windows 2003 SDK instead.  Do I
need to install the SDK or can I just pull the msitran.exe file from
the downloaded files?

So I downloaded and installed the Windows SDK the link pointed to.
Even though it said Vista SDK, it stated it worked for XP SP2, Vista
and Server 2003.

However, when I run the command line statement described in the
document, I get the following error:

Error 110. Error 2203: HIPAA33ForR2.mst, -2147287038

What am I doing wrong?

I tried using the MsiTran.exe that came with the Windows Server 2003
R2 SDK as well, but I get the same error!  What do I need to do to get
this transform to work?!?

That is why I was trying to get that MsiTran transformation to work as
prescribed in the document you listed in your earlier reply.  The
problem is the transform does not work!  Whether I use the MsiTran in
the Windows Vista SDK, which is the link specified in the document
mentioned above, or the Windows Server 2003 R2 SDK, I get the same
error and nothing happens:

Error 110. Error 2203: HIPAA33ForR2.mst, -2147287038

There is almost no documentation whatsoever regarding this error or
what to do about it so you can imagine how frustrating the last few
days have been.  I eventually tossed in the towel and uninstalled R2
and installed plain 2006.  I have yet to upgrade to R2, but that is
not out of the question since the documentation all states the upgrade
should be far smoother than the MsiTran route.  However, I would still
like to find out what I could have done, if only for my academic
curiosity.

Correct, as documented, it will not work.

This is for an upgrade only, not a new install.
