It works its way through the store through to completion and then starts again. You are going to see odd activity for several cycles. And then of course there will always be a few messages that Exchange doesn't think it needs to delete because it considers Outlook has modified it. Give it quite a few cycles and then look at the exceptions. If you're into a dozen or so after a week then I'd be content at that.
