How is bandwidth managed in cached mode?   
Lets say that hte following is true:
1 Central exchange 2003 server
300 Outlook 2003 clients distributed over 40 remote sites
All outllok 2003 clients are configured for Full caching only
I user sends a 10mb message to the Everyone dstribution list (all 300 users)

The question:
How would exchange or client handle this problem so that all of the clients 
are not receiving the 10mb message at the same time and using up all the 
bandwidth for some period of time?

Mark is right. Actually it is Outlook client that causes this problem but 
not Exchange. For the Exchange performance issue, I recommend reading this 
white book,

And my suggestions are as below:

1. Limit the message size in Exchange if your server hardware and network 
bandwidth is not powerful enough.

2. Configure Outlook client to download header for larger message. This 
option only applies to offline mode, so the workaround is as below:

Set Exchange Server account to work offline 
===============================		  
1. Start Outlook if it is not currently running.
2. On the Tools menu, click "E-mail Accounts".
3. Click "View or change existing e-mail accounts", and then click Next
4. Double-click the Microsoft Exchange Server account to access its 
properties.
5. Click the More Settings button.
6. On the General tab, click to select the "Manually Control Connection 
State" check box.
7. Under "Default connection state", click to select the "Work offline and 
use dial-up Networking" check box. You can also click to select the "Choose 
the connection type when starting" check box so that you work either 
offline or online.
8. Click OK.
9. Click Next, and then click Finish.

Configure Exchange Server account to download only headers for items larger 
than XXX KB 
===============================	
1. Start Outlook.
2. On the Tools menu, click Send/Receive -> Send/Receive Settings -> Define 
Send/Receive Groups.
3. Make sue to click the All Accounts group name (or the name of another 
Send/Receive group that you will use while you are working offline), check 
the "Schedule an automatic send/receive every x minutes" box under "When 
Outlook is Offline"
4. Double-click the All Accounts group name (or the name of another 
Send/Receive group that you will use while you are working offline).
5. In the Accounts bar, click Microsoft Exchange Server, and then click 
Inbox folder.
6. Select the "Download complete item including attachments" option and 
then check the "Download only headers for items larger than XXX KB" box.
7. After you set the other options as desired, click OK, and then click 
Close.
8. Restart Outlook, and then select to work offline.
