Change Password with OWA - error 5   

I have implemented the steps necessary to allow people to change their 
password using OWA.   This works fine for people in house.  However if 
someone outside of our network connects using OWA and the password expired 
box appears, when they change get error number 5 as a result.  Has anyone 
seen this before and if so how do you fix it?
