The MX record needs to point at some public address. That public
address has to be applied to a perimeter box that forwards TCP 25 to
your SBS box. The SBS box has a single NIC and one, single, internal
IP address. Go and do that then check that the firewall allows TCP 25
into the network and through to the server.
The outbound all depends on your DNS lookups and whether or not you
are routing through the ISP relay or not. Since you have a fixed IP
address you're probably delivering direct.
Learn about nslookup and do one against microsoft.com (for no
particular reason) to see if you're resolving. THen try telnet to port
25 of one of the results.
