This still doesn't answer the question of how to get the DOUBLE opening smart 
quote to face in the correct direction. It addresses single opening smart 
quotes and double closing. Anyone have the answer???

Sorry, the second "closing" below should be "opening":

Ctrl+`, " = opening double quote

That's what I get for cutting and pasting! Note, however, that all these 
keyboard shortcuts are given in the Insert | Symbol dialog (and also in 
http://word.mvps.org/FAQs/General/InsertSpecChars.htm)
