Why does VS2008 rebuild vc90.idl everytime?

I have a VS2008 managed CPP/ATL 9.0 project that rebuilds vc90.idl everytime
I build. This usually indicates that something as changed in one of the
dependancies for vc90 but I can't find any references in my project to
vc90.idl.
Where do I start looking?

how to disable menu keys

I would like to remap my keys in vsnet 2008 but some of the key
combinations conflict with menu combinations. As an example I would like
to map alt-w to save but the combination always opens the windows menu
list. Is there a way to over-ride this behavior?

Whenever I open Help (Help -> How Do I) from inside Visual Studio 2008,
dexplore.exe takes up about 50% of the CPU and never stops. If I close help,
the process still continues to take up 50% of the CPU. Even if I close the
whole of Visual Studio 2008, dexplore.exe is not killed.
