Missing Categories UI from Add New Item dialog

I'm using 2005 and noticed that my "Add New Item" dialog ONLY has the
standard templates and I do NOT even have the tree control to the left
that shows the categories. I noticed that other people with the same
install have this category tree control and see all of the 3rd party

Thanks for reviewing my question. I have VS 2008 and SQL Server 2008 CTP
installed. When I use the Server Explorer in VS, I am able to see my tables
but when I drill down to see the columns or open the table definition I get
