Setup question

In my vb.net solution I have added a setup project. What I am not sure is
how to configure setup so it does below when it is run;
1. Install the app in C:\Program Files\Company Name\Product Name folder.
2. Create a menu entry in start menu as Start->Product Name.
3. Create a desktop icon to app.

donlaoded the realteck 64 drivers however I get an error messasage (failure

Error in accessing the application after delpoying on server!

My application works fine on developmet server,but when I deploy this
application usitn VS websetup then try to access then this error
occurs.
Error:

I have IIS server 6.0 and dotnet framwork 2.0 on my server machine. I
intall the setup of my application to server machine Now I try to run
my website from server machine then its shows the html content (text
file) on the browser.
Why browser doesnot reder this Html? please tell me where is poblem?
