When to install an Exchange Server

I have look through all of Microsoft documents and emails from other groups
and no clear best practices. What would it take to have a centralize
Exchange Server compared to decentralized? I mean what is acceptable
bandwidth, most remote offices have 12 or less users, 1 has 60 and another

SMTP connector and Local Bridgeheads

If I specify two or more servers under Local Bridgeheads in the SMTP
connector. Is it possible to force Exchange to prioritize one server and only
use the other servers if the first one fails?

Exchange 2003 with Cache Mode in consolidation
