I have created an outlook form with a few checkboxes and radio buttons...I
would now like to publish this form into a MailBox and then be able to link
the controls placed on the form to a database (Access/SQL)...

Custom Forms and OWA

Hello.
I developed a custom Appointment form in Outlook 2003 and published it to
the Organizational Forms Librar. We are running Exchange Server 2003.
What do I need to do so that OWA uses the new form? I added a new
appointment to my Calendar folder using the new form, which shows fine in
Outlook, but ...

form in Outlook 2007. In Outlook 2003 the contact icon appeared on the
