Form collection

Hi Guys,
I would like to:
design a form to collect data
send it out via outlook
Have the receipient open it in a browser
complete the form
then have the summary data page emailed back as an attachment.
My question is what microsoft product/format will give me this functionality?

I packaged an Access 2000 program using 2000 Developer on my Vista Premium
OS. I sent to someone who tried to install it on a Vista Premium computer
and got the following error:

mouse scroll left to right & office 2007
