Form Not Working - Server Extensions

I have a contact form located at:

The website was published via FrontPage 2002 via:
File > Publish Web
My hosting account has FrontPage extensions installed. When I got the error
(shown below), I reinstalled the FP extensions just for good measure.

Hi all,
I am dealing since more than 2 weeks with my ISP because I am not able
anymore to publish with FP.
I have captured these 2 traces 1 is the POST from FP and the other is the
answer from the server.
Could someone please understand what happen?

e-mail address in the form properties box it tells me the extension is not
correct I am using Front Page 2003. It worked when I used Front Page 2002.

Being very, very careful:
When using FTP on a FrontPage extensions site:
1) Only move a single file at a time
2) NEVER move a folder. If you need a new folder on the site, create it
with the FTP program, do NOT upload it.
3) Do NOT even think about moving a folder or file with a name starting
with _vt ...

I have been trying to make a form that works, without installing server
extensions - because it sounded as though it was restrictive... and lots of
'what you won't be able to do' etc. if you do instal them.
One thing it said was that you will not have any .htaccess files at all - is
this true and what disad ...

Please can anyone tell me how to make a simple questions form just like the
