CC Field Populated via a Dropdown List

This has been explored before (see link below) but I tried to take the code
on the attached and manipulate it for my purposes and it didn't work.
My goal is to have a dropdown list that automatically populates the cc field

custom form not working in outlook 2000 in vista.

My custom form was working in outlook 2000 in XP. When I run from outlook 2000
in Vista, I get this error: "An error occurred registering the form in the
OLE registry". Everything else in Outlook works fine in vista for me except

Field Chooser on Customer forms items empty

when I open the field chooser in a custom Outlook form and select the
current custom form, there are no items listed in the list. There are
plenty of customer objects on the form that should be available.
Any ideas why these customer objects are not showing up?
