Probably an easy question... I have made a form, but when I send it the
fields that I have checked off as check boxes or as drop downs do not display

OLE Registry Error with custom form

When choosing a custom form user gets message "Form selected could not be
displayed. Error occured registering the form in the OLE registry". User
has cleared the cache, re-started Outlook but is still getting this message.
Why is the user getting this message, and what is the fix?

Routing Form

open custom form

I created a custom form to record information (school attendance) and
convert that information to a journal note for a contact. What I
can't figure out is how/where to put a button or a menu item in
Outlook so I can access the info easily. the only way I can do it now
