Custom form could not be opened

Hi,
I have created my custom form at Tools->Form->design
Forms->Calender->ConfRooms
I could save this form alog with Appointment and Scheduling tabs. but when i
opened it from same or other machine it says "The Custom form could not be
opened. Outlook will use an Outlook form instead". Any help to save t ...

Activation errors

I am trying to activate Office Home and Student on my main computer. When I
loaded it on nmy laptop it said I could load in on +- 3 other computers.
However when I try to activate it the system will not accepr the number 0 or
letter O which is in my activation code. What can I do to fix this?

Notes Field - Using Functions

I'm using Outlook 2003. In the Notes field, I tried using the followng
Format(Date(),"mmmm dd, yyyy")
and it did not reflect the date. Do Functions work in notes? If note,
is there anyway to reflect a date other than typing it in manually?

Appointment, tracking who last modified it

Is there a way on my custom appointment form to always show "who" last
modified the appointment ? I was able to locate how to record "when" it was
last modified, but not "who" last modified it.
