Mailmerge with a csv file

I'm using Word 2007 to mailmerge a previously created document with a csv
file. When I try, I get:-
XXXXXXXX.docx is a mail merge main document. Word cannot find its data
source, Y:\xxxx_data.csv.
Note the filename is exactly 16 characters long and always the same name.

I am trying to do a mail merge to create envelopes and everytime I select the
excel spreadsheet to use it changes the format of the zip code to 4 digits
instead of 5. How do I keep this from happening so the zip code remains 5
