Cannot find merge document

I have a word file that merges with a file on a mapped drive. When I open the
file it ask if I want to open a document with the following command:
SELECT * FROM (filename)
This used to work fine. However, for the last couple of weeks it gives me an

I am creating a form for wedding rehearsals. When I lock the form and use
it, the text moves over the next "item". For example:
Name: text goes here Date: text goes here
When I type in the name, the entire date section moves over. How do I

Track insert text only?

Is there a way of tracking only text inserted and not deleted text? I tried
going into tracking 'show', options put none for deletes, but it still shows
up.. (I have already taken off 'comments' and 'formatting'...) ...

This place has been amazing for help. So I am on to my next challenge.
Basically in a locked form view, an agent selects whether it is an eligible
claim, customer and/or product in check boxes. My goal is to make it look
like this:

How do I restore the "Customize ... " option to View > Toolbars me

word is there but greyed out.
